I've had about four of these by now, first one maybe 15-18 years ago. I can't imagine anyone willing to have one done without anesthesia, since anything involving direct and even peripheral contact with a nerve would undoubtedly be agonizing.
The pain for me, for the two most recent ones, was the cost-- Over $1000 just for the procedure, then another $700 to $900 for the crown you need to have installed afterward. Back in 2015 I had to have a tooth extracted simply because I didn't have the money at the time to save it. (Uncomplicated extractions are a mere $250 to $300).
